Minority Supplements to Institutional Clinical Oncology Research Career Development Award (K12)

 

PLEASE USE THE FOLLOWING FORMAT FOR REPORTING
THE PROGRESS ON THE MINORITY SUPPLEMENT
"K12 CURE"
INCLUDE WITH AND CLEARLY IDENTIFY IN THE
NON-COMPETING REPORT OF THE PARENT GRANT

 

A. A budget page for the coming year using standard form 2590.

B. The name and ethnicity of the appointee.

C. A brief paragraph describing the research and didactic training experiences completed and ongoing.

D. Future plans for satisfying the (core) requirements of the individualized career development plan for the trainee.

E. List of publications and/or clinical protocols resulting from their work in the Program. Include a description of the role of the trainee in each of the listed clinical protocols.

F. Other activities related to the Program

G. An Advisory Committee report that is separately attached providing an evaluation of the performance of the trainee during the reporting period, the progress of the trainee towards meeting the (core requirements) objectives of the overall K12 program and the individualized career development plan. Provide recommendations for the next funding period including changes in mentor(s), (core) requirements, etc.

H. Updated Personal Data Sheet for Minority Candidate
